What We Need From You
OCF-1 – Application for Accident Benefits
This form starts your auto insurance claim
OCF-3 – Disability Certificate
(if needed) Completed by one of our licensed practitioners
(e.g., chiropractor, physio) to confirm injury and treatment needs
OCF-5 – Permission to Disclose Health Information
Gives us permission to share your health info with your auto insurer.
Required to begin processing claims
Claim Number
Provided by your auto insurance company after you report your accident
Policy Number
Found on your auto insurance slip or policy documents
Auto Insurance Company Name
The company you’re insured with (e.g., Intact, Aviva, TD)
Adjuster’s Name & Contact Info
Your insurance adjuster is your main point of contact for your claim
Date of Accident
Needed to file claims and submit documentation
Copy of Driver’s License or Valid ID
Required to confirm your identity
Extended Health Insurance Information (if you have it)
We must bill any private/work benefits first before auto insurance, as per Ontario law
Signed Consent Forms (from our clinic)
Required for treatment and claims submission
How Billing Works
(No Out-of-Pocket Payments)
We follow block fee billing, usually broken into treatment blocks over 12 weeks (3 months)
Your private insurance is billed first for eligible services
(e.g., physiotherapy, chiropractic)
Any remaining unpaid balance is submitted to your auto insurer
You don’t pay out of pocket — we coordinate all billing directly with insurers
Important Notes
All treatment and services must be rendered at our clinic — outside treatment is not covered through your plan with us.
We’ll work with your auto insurer to manage approvals, documentation, and communication.
You may receive calls or mail from your adjuster — feel free to bring anything to us if you’re unsure how to respond.
We do not offer Massage Therapy under Motor Vehicle claims.
